IVIG (Intravenous Immunoglobulin)
What is IVIG (Intravenous Immunoglobulin)?
IVIG stands for intravenous immunoglobulin. It is not one brand of medicine. It is a type of treatment sold under several different brand names, such as Gamunex-C®, Privigen®, Panzyga®, Octagam®, and Gammagard Liquid®.
Each brand is a little different. They treat different conditions, run at different speeds, and carry their own warnings. Your provider will work with your insurance plan to determine which brand is covered.
In neurology, IVIG is used most often for a nerve condition called CIDP. It is also used for multifocal motor neuropathy, dermatomyositis, and sometimes for myasthenia gravis.
Here is what treatment usually looks like:
- Your dose: Based on your weight and your condition
- How often: Usually every 3 to 4 weeks, after a starting course of treatments
- Total appointment time: Usually 3 to 5 hours, though this varies a lot by dose and by brand
Your nurse will start your treatment slowly and speed it up as you get comfortable. If you get a headache, the nurse can slow it back down. Drinking plenty of water in the days before your visit helps a lot.
How does IVIG (Intravenous Immunoglobulin) work?
Antibodies are proteins your immune system makes to fight off germs. IVIG is made from antibodies donated by thousands of healthy people.
In some nerve conditions, your body makes antibodies that attack your own nerves by mistake. This causes weakness, numbness, and tingling.
Donated antibodies help in more than one way at the same time. Doctors do not think there is one single switch that IVIG flips.
Here is what it does:
- It crowds out the harmful antibodies. Donated antibodies help your body clear the ones attacking your nerves.
- It calms part of your immune system. This lowers the swelling around your nerves.
- It speeds up antibody cleanup. Your body clears out harmful antibodies faster.
- It quiets immune cell signals. This lowers the overall immune response.
How fast IVIG works depends on your condition. In CIDP, many people notice their strength improving over several weeks.
Your doctor may change how often you get treatment based on how long the benefit lasts for you. If you start feeling weaker before your next visit, tell your care team — that information helps them adjust your schedule.
Common side effects:
- Headache — this is the most common one, and it often happens when the treatment runs too fast
- Fever, chills, or feeling flushed during treatment
- Feeling tired
- Upset stomach
- Sore muscles or joints
Serious side effects — get medical help right away:
- Blood clots. IVIG can cause blood clots. Your risk is higher if you have heart problems or if you sit still for long periods. This is one of two boxed warnings for IVIG. A boxed warning highlights severe potential side effects, dangerous drug interactions, or critical usage restrictions.
- Kidney problems. IVIG can damage your kidneys, and in rare cases cause kidney failure. Your risk is higher if you already have kidney disease. This is the second boxed warning.
- Swelling around the brain. This can cause a bad headache, a stiff neck, fever, and trouble with bright light. It usually starts within a day or two of treatment.
- Breakdown of red blood cells. This can make you feel very tired or short of breath.
- Serious allergic reactions. These are more likely in people who are low in a protein called IgA.
